yani www.site.com/from.php yi açınca www.site.com/404.php gözüksün
ama www.site.com/index.php?dosya=from.php olunca yada index.php üzerinden linke tıklatılınca form.php nin açılmasını istiyorum acil

Büyük ihtimal form.php yi include ediyorsun. Basit bir şekilde form.php yi include etmeden önce anahtar bir değişkene bir değer ata ve öyle include et.
form.php den de eğer o değişkende o değer varsa açsın yoksa yönlendirsin.

index.php:
$anahtar='abc';
include('form.php');

form.php:
if($anahtar != 'abc')
header('Location:404.php');

javascript:?? host firması kendi hata sayfanı oluşturmayı destekliyordu diye tahmin ediyorum panelin özelliği olarak o sayfayı değiştreiblirsin. 404 sayfa bulunamadı hatası değişik ifadeler de buradan bulabilirsin.
